Peroral Endoscopic Myotomy (POEM) is a minimally invasive endoscopic procedure used to treat achalasia, a condition that affects the esophagus. This technique involves making small incisions inside the esophagus through an endoscope to cut the muscle fibers that are causing the obstruction. The goal of POEM is to relieve the symptoms of achalasia, such as difficulty swallowing, by improving the function of the lower esophageal sphincter.
POEM is typically recommended for patients with achalasia who have not found relief with other treatments, such as medication or balloon dilation. It is used when symptoms are severe and affect daily life, and when other less invasive treatments have been ineffective.
POEM is performed endoscopically, meaning there are no external incisions. The procedure typically takes 1-2 hours and is performed under general anesthesia. During the procedure, a flexible endoscope is passed through the mouth and into the esophagus. A small incision is made in the lining of the esophagus to access the muscle layer, where the targeted muscle fibers are cut to improve the function of the esophagus. Afterward, the incision is closed, and the patient is closely monitored before discharge, usually within 24 hours.
Recovery from POEM is typically quick, with most patients returning to their normal activities within a few days. Patients may need to follow a modified diet (starting with liquids and soft foods) for a short period after the procedure to ensure proper healing. Most patients experience significant improvement in swallowing and other symptoms soon after the procedure. POEM has a high success rate and is an effective, long-term solution for esophageal motility disorders.
Untreated achalasia or esophageal motility disorders can lead to significant complications, including difficulty swallowing, regurgitation, and malnutrition. Over time, this may result in an increased risk of aspiration (food or liquid entering the lungs) and esophageal dilation, requiring more complex treatments.
If you are experiencing difficulty swallowing or have been diagnosed with achalasia, Peroral Endoscopic Myotomy (POEM) may be the right treatment for you. POEM has a high success rate, with many patients experiencing significant improvement in symptoms.
POEM is designed to improve swallowing by relieving the obstruction in the esophagus, leading to better overall function.
POEM is primarily used for achalasia, but it may also be considered for other conditions affecting esophageal motility in some cases.
Many insurance plans cover POEM, but it's important to check with your insurance provider for specific coverage details.
The procedure typically takes about 1 to 2 hours, depending on the complexity of the case
